The second paper was based on the challenges that are faced during the implementation of information security management. Both the peer reviews were concluded with useful recommendations that can facilitate improvement of the paper; such recommendations bear great relevan ce in the process of conducting peer reviews. However, one factor that has been missed out from the peer reviews is that of the mention of the authorsââ¬â¢ details and the place where this paper was presented. 3. Most Challenging Submission The article writing proved to be the most challenging and difficult submission in the course. The topic of my research was ââ¬Å"The Relation between IM, Information Security and HRMâ⬠; all the three aspects of this chosen domain involve extensive research since numerous papers and literature are available for each one of them. The excessive nature of the available literature made me lose focus at few instances during the research process and made me include information that could not prove to be highly relevant for the research topic. The inclusion of too much information proved to make the paper generic in nature to a certain extent rather than focusing on the link of information systems, information security management practices and h uman resource management practices. Meeting the word limit (i.e. 3000 words) of the paper also proved to be a challenge that led to the compromise of the quality of the content. Role of Ammonia in Hepatic Encephalopathy Essay The human liver is truly amazing, however little is ever said about the organ. Most people probably wouldnââ¬â¢t be able to locate their liver if theyââ¬â¢re asked to do so. The second largest organ in the body and weighing about 3 pounds, the liver is located right under your rib cage on the right side of your body. It is the only organ that is able to regenerate. While most organs replace damage with scar tissue, the liver has the ability to replace damaged tissues with new cells. The primary function of the liver is to process food that has been consumed into nutrients and filter out any harmful substances that may be the bloodstream. The liver is responsible for keeping our blood suitable for our bodies. With that understood it saddens me to say that there is an estimated 30 million people living in the U.S. with liver failure. These 30 million people are unable to filter out any harmful toxins that may be flowing in their bloodstream. With time they may began to suffer from Hepatic Encephalopathy. Described as episodes of confusion and altered levels of consciousness Hepatic Encephalopathy is caused by to an accumulation of ammonia in the bloodstream that would usually be filtered out by the liver. Although Hepatic encephalopathy is completely reversible with treatment there are very few treatments available. Pharmaceutical have saved many lives through experimental studies and treatments. Although very controversial; many people choose to participate in these studies in hopes of increasing their chances to live a normal life despite suffering from an ailment. I chose to do my assignment on a double-blinded study on the use of Glycerol Phenylbutyrate (also known as Ravicti) in patients with Episodic Hepatic Encephalopathy. The study was completed by Dr. Don Rockey who is the Chief of Medicine at the Medical University of South Carolina. While delivering his oral presentation which he titled ââ¬Å"Randomizedâ⬠, Dr. Rockey mentioned ââ¬Å"the study results provide new insight into the importance of ammonia in the pathogenesis of hepatic encephalopathy. Based on its safety profile, Hyperionââ¬â¢s investigational drug, Glycerol Phenylbutyrate shows promise as a novel therapeutic agent.â⬠The study was conducted with a total of 178 patients selected randomly. 50 of who were on the drug Rifaximin which is a drug also commonly used to prevent episodes of heptatic encephalopany. 29 of the 178 patients were placed on placebo and the remaining 30 patients were placed on the active drug Ravicti. If effective those on the active drug Ravicti should expect to see fewer levels of ammonia in the blood, thus resulting in fewer episodes of heptatic encephalopany. Ho = Patients placed on Ravicti will see no change in ammonia levels in the bloodstream and will continue to have Hepatic Encephalopathy episodes. H1 = Patients placed on Ravicti will see change in ammonia levels in the bloodstream and will have little to no Hepatic Encephalopathy episodes. The results from the study conclude that among the 119 patients not on Rifaximin showed a highly statistical significant reduction among treated patients with 10% vs. 32% (p= 0.003.) Among the patients in the active part of the study experienced fewer Hepatic Encephalopathy episodes 21% vs. 36% (p=0.021.) Those in the active study also reported ammonia levels to be significantly lower 45.7 vs. 58.15 umol/L, (p=0.0036) (p 0.01). A concern I noticed while researching this study is the accuracy of the statistical results since this study was two sided this means that curtain results were expected by both the test subjects and its administers. This study was also done on a small population of people, which is a bias in itself since the sample population lacks diversity. Factors like diet, environment and previous medications were not mentioned in the study which also can contribute to hepatic encephalopathy episodes. Another key factor that is important to gaining a reasonable sample through population is a correlation between test subjects. Thyroid Gland And Thyroid Hormone Synthesis Hypothyroidism is a condition characterized by abnormally low amount of the thyroid hormone synthesis. This may be due to a thyroid problem or any other reason. Thyroid hormone affects growth, development, and many cellular processes. Inadequate thyroid hormone has many consequences for the body Thyroid gland and thyroid hormone synthesis Thyroid gland has two lobes connected by an isthmus. It attaches to the thyroid cartilage and trachea. Therefore it moves with swallowing. Thyroid gland consists of follicles lined by a cuboidal epithelial cell layer. These follicles filled with colloids.Parafollicular cells situated in between follicular cells which secrete calcitonine. Thyroid gland synthesizes mainly two hormones. They are L-thyroxin/tetraiodothyronine (T4) and triiodothyronine (T3).T3 is the active form that acts at the cellular level and T4 is the prohormone.Iodide enters the thyroid follicles primarily through a transporter. Thyroid hormone synthesis occurs in the follicular space through a series of reactions, many of which are peroxides-mediated. Thyroid hormones stored in the colloid in the follicular space that is released from Thymoglobulin by a hydrolysis reaction which occur inside the thyroid cell. E.g. Thyroglobulin (Tgb), monoiodotyrosine (MIT), diiodotyrosine (DIT), Triiodothyronine (T3), tetraiodothyronine (T4). If there is hereditary defect of enzyme in above process, Thyroid hormone synthesis could not occur. So its leads to congenital goiter and often results in hypothyroidism. Regulation of the synthesis of thyroid hormones Thyroid hormone synthesis is regulated by another gland located in the brain called pituitary. In turn the pituitary gland in part regulated by the thyroid hormone via feedback mechanism and other gland called hypothalamus. The hypothalamus secretes thyrotropin hormone (TRH), which give a signal to the pituitary gland to release thyroid stimulating hormone (TSH). TSH in turn sends a signal to the thyroid gland to release thyroid hormone. If some defect occurs in one of these levels, a lack of production of thyroid hormones can cause a deficiency of thyroid hormone (hypothyroidism). Hypothalamus TRH down arrow Pituitary- TSH down arrow Thyroid- T4 and T3 The rate of thyroid hormone synthesis is regulated by the pituitary gland. If there is an insufficient amount of thyroid hormone circulating in the body to normal functioning, the release of TSH from the pituitary increased in order to stimulate more thyroid hormone. However, when there is a large amount of thyroid hormones in circulation, TSH level decreases and pituitary attempts to reduce the production of thyroid hormone. In people with hypothyroidism have low levels of circulating thyroid hormones. Physiological effects of thyroid hormones à ¢-à Cardiovascular system increased cardiac output and heart rate. à ¢-à skeletal system increased bone turnover and resorption. à ¢-à Respiratory maintains normal hypoxic and Hypercapnic drive in respiratory centre. à ¢-à Gastrointestinal increases gut motility. à ¢-à Blood increases red blood cell 2, 3-BPG facilitating Oxygen release to tissues. à ¢-à Neuromuscular increases speed of muscle contraction and relaxation and muscle protein turnover. à ¢-à Metabolism of carbohydrates increases hepatic Gluconeogenesis/glycolysis and intestinal glucose Absorption. à ¢-à Metabolism of lipids increased lipolysis and Cholesterol synthesis and degradation. à ¢-à Sympathetic nervous tissue increases catecholamine Sensitivity and ÃŽà ²-adrenergic receptor numbers in heart, skeletal muscle, adipose cells and lymphocytes. Reduces cardiac ÃŽà ±-adrenergic receptors. If there is a defect in the synthesis or regulation pathways or thyroid lead to many disorders. They are mainly divided in two parts. They are hyperthyroidism and hypothyroidism. Hyperthyroidism, or an overactive thyroid, is the overproduction of thyroid hormones T3 and T4, and most often caused by the development of Graves disease which is an autoimmune disease in which antibodies are produced which stimulate the thyroid gland produces excessive amounts of thyroid hormones. This disease can lead to the development of toxic goiter due to the growth of the thyroid gland in response to the absence of negative feedback mechanisms. This is manifested by symptoms such as thyroid goiter, protruding eyes (exopthalmos), palpitations, excessive sweating, diarrhea, weight loss, muscle weakness and unusual sensitivity to heat.
